The $500 deposit is non-refundable. By making this payment, the patient agrees that if they do not show up for their scheduled appointment or procedure, they forfeit any right to request a refund.
This same policy applies to any additional payments made. If the patient does not attend their appointment or arranged procedure, Smiles by Mario Montoya Atlanta reserves the right to retain up to 10% of the total amount paid to that point.
In cases of emergencies or an unexpected change of events, the patient may request to reschedule their appointment, but the deposit made will still be non-refundable, and we will retain 10% of any additional payments.
